Life and Disability with Mutual of Omaha

Homeowners provides a $50,000 life insurance policy to each employee, at no cost. 

You are able to purchase  additional life insurance for yourself (up to $500k), spouse/domestic partner (up to $250k), and child dependents (up to $10k). 

Employees can elect Short Term Disability coverage, payable for up to a maximum of 12 weeks (depending on type of disability) and is calculated at 60% of your covered earnings up to a $2,000 weekly maximum

Employees can elect Long Term Disability coverage, calculated at 60% of your covered earnings.  There are two tiers of Long Term Disability payments ($5,000/$10,000) - based on earnings eligibility.
